Pike County: (RFQ) Construction Management Services in connection with the Prussia Road Slip Repair Project, PIK-CR46-01.11. Engineering firms interested in being considered for a contract to provide the required services should reply with a statement of qualifications no later than 4:00 P.M. on Friday, May 13, 2022. Statements received after this deadline will not be considered. The following rating factors will be used by Pike County Engineer to evaluate each statement of qualifications. All rating factors will have equal relative importance. Please respond only to the questions listed. We request that you do not send additional forms, brochures, or similar material. 1. List the type of work for which your firm is currently prequalified by the Ohio Department of Transportation. 2. Detail the project staffing, broken down by the number of: a. Professional Engineers b. Technicians c. Other Specialists Also indicate the number of such personnel available for assignment to this project. 3. List the project manager(s). List name(s) of field personnel likely to be assigned to the project. Attach resume(s). 4. List the experience of your firm's personnel on similar projects. 5. Provide a representative listing of projects similar to the proposed project performed by your firm. Include number of years your firm has been in business. 6. List your firm's present workload. Please specify the dollar value of PRESENT OUTSTANDING fee agreements. 7. Provide references. 8. Provide a brief description of your firm's equipment and facilities. Statements of qualifications should be transmitted to The Board of Pike County Commissioners c/o Denny T. Salisbury, P.E., P.S., Pike County Engineer, 502 Pike Street, Waverly, Ohio 45690. As required by Ohio Revised Code, Section 153.65-71, responding firms will be evaluated and ranked in order based upon their qualifications. Following this evaluation, the Board of Pike County Commissioners will enter into contract negotiations with the most highly rated firm. Place qualified small and minority businesses and women's business enterprises on solicitation lists. 2. Assure that small and minority businesses and women's business enterprises are solicited whenever they are potential sources. 3. Divide total requirements, when economically feasible, into smaller tasks or quantities to permit maximum participation by small and minority businesses and women's business enterprises. 4. Establish delivery schedules, where the requirement permits, which encourage participation by small and minority businesses and women's business enterprises. 5. Use the services and assistance, as appropriate, of such organizations as the Small Business Administration (SBA) and Minority Business Development Agency of the Department of Commerce (DOC).
