(08A3422) CALTRANS: w/ *ADDENDA 1-2* Unsheltered Resident Encampment Site Waste Removal and Cleanup in Riverside County. Contractor agrees to provide Unsheltered Resident Encampment Site Waste Removal and Cleanup services to the California Department of Transportation (Caltrans) on an on-call, as-needed basis. The Prime Contractor or Subcontractor must have, at time of bid submittal and for the duration of the contract, a valid, current: A. Contractor License(s): Class A General Engineering license and a HAZ - Hazardous Substance Removal Certification from California Contractor¿s State License Board (CSLB). B. Hazardous Materials Transportation License issued by California Highway Patrol (CHP). C. Hazardous Waste Transporter Registration issued by the California Department of Toxic Substances Control. D. Hazardous Materials Certificate of Registration issued by U.S. Department of Transportation Pipeline and Hazardous Materials Safety Administration. D. Trauma Scene Waste Management Practitioner (from State of California Department of Public Health, Medical Waste Management Program). E. Waste Tire Hauler Registration issued by Cal Recycle. F. Hazardous Waste Operations and Emergency Response (HAZWOPER) Certificate of 40 hours of training. G. Confined Space Entry Certification, as required by the Department of Industrial Relations, Division of Occupational Safety and Health (Cal/OSHA), in Exhibit A, Section 17. A permit and contractors program documents are required at the time of bid submittal. H. Contractor or subcontractors applying or spraying disinfectant shall possess a Pest Control Business (PCB) license and have a Qualified Applicator License (QAL) with Category K (health related) staff to oversee the business operations. Both of these licenses are issued by Department of Pesticide Regulation. https://www.cdpr.ca.gov. Contractor or subcontractors applying or spraying disinfectant shall also be registered with each individual county agricultural commission where the disinfectant will be applied (Los Angeles and Ventura counties). Proof of these requirements shall be provided to Caltrans Contract Manager prior to applying or spraying disinfectant. Bidder and any subcontractor(s) shall possess at the time of bid submittal and for the duration of the Agreement, a valid and current Motor Carrier Permit issued from the California Department of Motor Vehicles (DMV) as described in Item 7 of this Invitation for Bid and in Exhibit E of the Proposed Forms of Agreement, Attachment 7. Bidder and any subcontractor(s) shall possess at the time of bid submittal and for the duration of the Agreement, a valid and current registration with the Department of Industrial Relations (DIR).
